Error with form - missing fields

Required fields are fm_handle and fm_redirect
Semi-required fields are fm_email, fm_name and fm_subject
Optional field is fm_cc

If you are a site manager and are having trouble using this cgi script, contact , otherwise, contact the site manager and let them know they have a broken form.
